Program Terms and Conditions

Participants in the Tandem Upgrade Program ("Program") must be able to cover the costs of supplies for the duration of their original pump's existing warranty ("Program Period"), either through their current insurance plan or other standard payment options. Purchase of supplies directly from Tandem is not required; in most cases participants can continue to purchase supplies from their current distributor. Insurance verification, including confirmation of duration of the participant's current pump warranty, and a prescription will be required to start the Program. Payment plans are not available in connection with the Program.

Program terms and conditions are subject to change at any time, without notice. Limitations apply. A valid prescription is required. This offer is not available for people under 2 years of age. Offer is not valid for: members of a Kaiser Permanente Health Plan; members of health plans or insurance contracts that prohibit this Program; residents of any state where prohibited by law; persons residing outside the United States and Puerto Rico or where otherwise prohibited; customers on a Tandem payment plan; and customers using a pharmacy benefit for a pump.

Customers Switching to Tandem from a Non-Tandem Pump

The Program is available for non-Tandem pump users with at least six months remaining on the warranty of their current pump to switch under the Program for a program fee of $999. Offer not available to individuals with in-warranty pumps who are eligible for a new pump through their insurance plan or for any person insured or subject to reimbursement in any party by Medicaid, Medicare, Tricare, or any other federal or state health programs. The remaining warranty duration of participant's existing pump will apply to the Tandem pump received through the Program. Warranty support for the Tandem pump during the Program Period shall be in accordance with the warranty accompanying the Tandem Pump.

Customers switching to a Tandem pump from a non-Tandem, in warranty pump will have 90 days from receipt of the Tandem pump to determine if they prefer the Tandem pump or their current, non-Tandem pump. Customers who decide to return the Tandem pump within the 90-day return period are eligible for a refund of the Program fee minus a $199 administrative fee. Customers who decide to keep the Tandem pump must return the Tandem pump at the expiration of their non-Tandem pump's warranty. Tandem is unable to guarantee returns of pumps or supplies purchased through a DME distributor. Tandem and cannot accept returns of pumps or supplies purchased through a pharmacy. Failure to return the Tandem pump within 90 days of the warranty expiration will result in a charge for the full cash price of the Tandem pump, less the prior one-time payment of the Program fee.

Indications for Use

Tandem Mobi system: The Tandem Mobi insulin pump with interoperable technology (the pump) is intended for the subcutaneous delivery of insulin, at set and variable rates, for the management of diabetes mellitus in persons requiring insulin. The Pump is able to reliably and securely communicate with compatible, digitally connected devices, including automated insulin dosing software, to receive, execute, and confirm commands from these devices. The pump is indicated for use in persons 2 years of age and greater.

t:slim X2 insulin pump: The t:slim X2 insulin pump with interoperable technology (the pump) is intended for the subcutaneous delivery of insulin, at set and variable rates, for the management of diabetes mellitus in persons requiring insulin. The pump is able to reliably and securely communicate with compatible, digitally connected devices, including automated insulin dosing software, to receive, execute, and confirm commands from these devices. The pump is indicated for use in persons 2 years of age and greater.

Control-IQ+ technology: Control-IQ+ technology is intended for use with compatible integrated continuous glucose monitors (iCGM, sold separately) and alternate controller enabled (ACE) pumps to automatically increase, decrease, and suspend delivery of basal insulin based on iCGM readings and predicted glucose values. It can also deliver correction boluses when the glucose value is predicted to exceed a predefined threshold. Control-IQ+ technology is intended for the management of type 1 diabetes mellitus in persons 2 years of age and greater and of type 2 diabetes mellitus in persons 18 years of age and greater.

Warning: Control-IQ+ technology should not be used in anyone under the age of 2 years old with type 1 diabetes or under the age of 18 years old with type 2 diabetes. It should also not be used in patients who require less than a total daily insulin dose of 5 units of insulin per day or who weigh less than 20 pounds (9 kilograms), as those are the required minimum values needed for Control-IQ+ to operate safely.

Users of the pump and Control-IQ+ must: use the insulin pump, iCGM, and all other system components in accordance with their respective instructions for use. Failure to follow these instructions for use could result in an over delivery or under delivery of insulin. This can cause hypoglycemia (low BG) or hyperglycemia (high BG) events.
