* Team Tandem is open to in-warranty Tandem pump users who are at least 18 years old or parents and guardians of in-warranty Tandem pump users. † In compliance with applicable laws, healthcare providers will not receive program kits. Eligibility for benefits and promotions may be limited to in-warranty customers and cut-off dates may be implemented to allow for fulfilment to members who are qualified by a specific date.
