Infusion Sets
Returning Your Infusion Set
Please use the kit provided and follow the easy steps below to package the infusion set you are returning.
Plastic Container
Twist off the white lid from the plastic container and place the infusion set you are returning inside. Securely close the lid by twisting it clockwise.
Plastic Bag
Place the container into the clear plastic bag
Remove the yellow paper on the plastic bag to expose the adhesive strip
Fold the top of the bag over and press the adhesive strip against the plastic underneath to seal the bag
Shipping Box
Put the bag into the white shipping box.
If there is any excess plastic at the top of the bag, you might need to further fold the plastic so the bag can fit inside the box.
Close the lid of the white box and insert the small flap on the front of the box into the opening slit of the lid
Remove the paper backing from the clear, circular sticker provided in your kit.
Apply the top half of the sticker to the front edge of the lid and then fold the bottom half down to apply it to the bottom side of the box. This will ensure that the lid is securely sealed for transport.
Remove the paper backing from the provided shipping label and apply the label to the box lid. Press across the entire label to make sure it is securely attached.
Drop off the package at a UPS store or drop box
